Defining a vaccine breakthrough infection
... a vaccine breakthrough infection is defined as the detection of SARS-CoV-2 RNA or antigen in a respiratory specimen collected from a person ≥14 days after they have completed all recommended doses of a U.S. Food and Drug Administration (FDA)-authorized COVID-19 vaccine.
Identifying and investigating hospitalized or fatal vaccine breakthrough cases
As of May 1, 2021, CDC transitioned from monitoring all reported vaccine breakthrough cases to focus on identifying and investigating only hospitalized or fatal cases due to any cause. This shift will help maximize the quality of the data collected on cases of greatest clinical and public health importance.

Breakthrough Infections
The Conversation published an interesting article titled What is a breakthrough infection? which included this statement:-
When people get infected after vaccination, scientists call these cases “breakthrough” infections because the virus broke through the protective barrier the vaccine provides.[1]
The implication that the virus somehow breaks through a protective barrier created by a vaccine is misleading. In many cases the apparent breakthrough case is a result of a test but the person has no significant symptoms. The problem is described more thoroughly in the paper What should define a SARS-CoV-2 “breakthrough” infection?[2] which was quoted by The Conversation. Maintaining statistics of 'breakthrough infections' became meaningless. The only data of value is when a breakthrough infection results in symptoms that warrant hospitalization. And for some of those, the outcome is death.
COVID-19 Vaccination Breakthrough Deaths
The purpose of COVID-19 vaccinations is to survive infection, and vaccinated people who are challenged by the virus may get sick but can reasonably expect not to develop serious medical conditions and thus survive, thanks to the vaccine. There really isn't a standardised term yet to categorise cases where a person has been fully vaccinated for COVID-19, gets sick (breakthrough) and dies, for which the heading of this section may be an appropriate descriptor.
